Westminster referrals

This page provides links to all the existing referral forms for Westminster community services.

Referrers can contact the adult services Single Point of Access (SPA) team between 8am and 5.00pm from Monday to Friday.

Making a referral

Electronic referrals via SystmOne

The preferred method of referral into SPA is an electronic referral via SystmOne. We currently accept electronic referrals for the following services:

Email referrals

The following adult services currently accept referrals via email and efax:

  1. Identify the service to which you would like to refer
  2. Download the appropriate referral form from lists below
  3. Send the referral form to the tri-borough SPA team by:

Verbal referrals

The following adult services currently accept verbal referrals:

  • Referrals to our night nursing service are via 111 (8pm until 8am).

Pembridge Palliative Care Centre

Referral process to follow:

  1. Please add Pembridge Hospice to the list of service to the Single Point of Access Inner London Support
  2. Upload this referral form (PDF version and Word doc)
  3. Please add in bold if the referral is urgent and that the referrer should call Pembridge directly on 020 8102 5000 and seek advice.
