Merton referrals

Merton case management model

Introduced in February 2017, our case management model has a team of clinical and non-clinical case managers and care navigators, including a pharmacist, working together to meet the complex and multiple needs of adult patients in Merton.

We combine clinical and non-clinical interventions ie, social prescribing, to address medical, social, psycho-social, mental, Isolation, non-engaging, substance misuse and pharmacy/medicine issues. We also help clients that are not engaging with the process and have substance misuse issues or are homeless.

Verbal referrals

1. Identify the service to which you would like to refer
2. Call the Merton SPA: 0333 004 7555
3. Provide the referral details requested
4. GPs may be required to provide an EMIS summary
