Face to face structured education remains on hold, however patients are offered a virtual version of DESMOND called “My Desmond”. This is a Webinar Module for patients and is offered via the Book and Learn SWL System. |

Complex Case Management |

The service is accepting referrals for complex care patients (within the 1 day/5 day response). However to optimise resources, in line with unpredictable demand, the staff will work as 2 teams (North/South and East/West). The Virtual first approach will be followed, minimising unnecessary face to face contact. Priority will be given to patients:
Complex care MDT discussions (weekly) has resumed as a virtual offer from August. ECPs on the waiting list will now be seen from August. Weekly virtual MDT invitations will be booked and offered from September. |

All referrals are being clinically triaged to confirm urgency of priority and a telephone call will be made to patients to promote self-management/redirected to alternative support as appropriate. Capacity will be limited due to the service supporting Discharge to Assess pathways which have been implemented in response to COVID pandemic to ensure a quick, safe discharge for medically optimised patients. |
