Our rehabilitation units in Edgware and Finchley are for people who are assessed as being medically optimised and ready to leave acute hospital care.

Patients will have therapy input in the rehabilitation units to help them become as independent as possible. Basic nursing care is provided on the units and we aim to support patients to manage their own health conditions and look after themselves.  

Our 'decision to admit' (DTA) unit in Finchley helps provide medically optimised patients a short period of time to complete necessary assessments to develop their care plans to be able to discharge to an appropriate safe discharge destination.

Both our rehabilitation units and DTA unit has access to senior doctors from Monday to Friday, who will be available as needed. A consultant makes weekly visits to our rehabilitation units.

Out-of-hours medical cover is provided by a local GP service for our rehabilitation and DTA units. 

GPs and other clinicians from acute hospitals such as Royal Free, UCLH, Northwick Park can refer.

Inclusion

  • Patients who have rehabilitation needs
  • Patients who require additional assessment needs before being discharged to an appropriate destination

Exclusion

  • Patients that are not registered with Barnet GP
  • Patients that are medically unwell
  • Patients that are unable to participate in rehabilitation process due to level of cognitive impairment
  • Patients that have no rehabilitation goals
  • Patients under the age of 18
