The Learning Disability Nursing team provides:
- Complete specialist Learning Disability Nursing Assessments when identified as a need (e.g. complex health needs/behaviours that may be challenging) in addition to GP Learning Disability Annual Health Checks
- Learning Disability Nursing can facilitate and provide advice/support for individuals who may not be able to access generic health services i.e. requiring specialist assessments for desensitisation, reasonable adjustments in accessing primary and secondary health services, where a multi-disciplinary and multi-agency approach may be required
- Support individuals in arranging and attending health appointments where there is a specific need for a Specialist Learning Disability Nurse to attend
- Facilitate GP practices to improve uptake of health checks and health action plans, specifically for those individuals who may require additional support e.g. reasonable adjustments
- Ensure hospitals provide reasonable adjustments i.e. Hospital Passports
- Learning Disability Nursing is involved in developing and reviewing pathways in conjunction with Public Health and other health services, we also provide training and teaching to families, carers, staff and professionals who support people with Learning Disability
