Our service provides multi-disciplinary assessments and therapy for housebound adult patients with physical and cognitive needs.

Domiciliary therapies delivers a non-urgent planned care service, we see patients at home, in day-care, residential care and nursing home settings.
 

We are a team of physiotherapists, occupational therapists and assistive practitioners, and work collaboratively with our patients to identify individualised treatment goals to support and encourage patients with disabilities and long-term conditions to live independent lives.

We also support informal carers and family to manage the patient effectively within the home environment.

Inclusion

Exclusion

  • Patients who can attend outpatient services
  • Patients already being seen by a physiotherapist or occupational therapist in the community or outpatient setting to meet their current identified therapy needs
