My Care, My Way is a service that works in collaboration with a range of GP surgeries in Kensington & Chelsea and some parts of Westminster to provide holistic, joined up and proactive care to patients aged 65 years and older, and for those affected by the Grenfell fire. My Care, My Way supports you to navigate every aspect of your care by putting you at the heart of planning for your health needs, both today and in the future. We aim to provide a flexible and responsive approach to your care, and to reduce the risk of unplanned hospital admissions or emergency care.
We are a team of Case Managers and Health and Social Prescribing Coordinators who work with your GP to provide individualised care plans to all patients. We will work with family and carers, and will coordinate each patient’s care to ensure they are linked in with all appropriate services. Our focus is on planned care that anticipates and empowers you to prepare for and manage any changes in your health and social care needs. We have links with a range of services such as hospitals, social services, third sector and voluntary organisations, charities, District Nurses, and other teams within the NHS.
As long as your GP surgery works in collaboration with the My Care, My Way service, our inclusion criteria are that you must be 65 years of age or older, or have been affected by the Grenfell fire.
Referrals into the My Care, My Way service can currently only be made by your GP surgery.
We do not currently accept self-referrals.
