Cardiac Rehabilitation (Hertfordshire)

We are a multidisciplinary group of cardiology specialist health professionals including; physiotherapists, nurses, occupational therapists,
a dietitian, and clinical exercise specialists. We aim to help you regain your confidence and provide you with advice 2022-2023 Logo.jpgand support to allow you to get back to normal as quickly as possible.

We will also provide you with information and advice to help reduce the risk of further heart health problems.

Our service provision is delivered by members of the British Association of Cardiac Prevention and Rehabilitation (BACPR) accredited cardiac rehabilitation service based in West Hertfordshire.

Our cardiac rehabilitation service is for all patients who have been referred via primary care, GP, or Heart Failure Specialist Nurse with the following heart health concerns:

  • Myocardial Infarction, Segment elevation myocardial infarction (STEMI) or Non-ST-elevation myocardial infarction (NSTEMI) (Heart Attack)
  • Cardiac Revascularisation via Angioplasty or Percutaneous Coronary Intervention (PCI)
  • Coronary Artery Bypass Graft
  • Heart Valve Replacement or Repair
  • Heart Failure

We aim to call you within 1 - 2 weeks of you being discharged from hospital. We will discuss your progress; answer any queries you have and see how you are feeling both physically and emotionally. We will also explain our cardiac rehabilitation programme and invite you to attend an initial assessment appointment with a member of our multidisciplinary team.

West Hertfordshire offers three methods of delivery of cardiac rehabilitation. These methods include:

  • Face to Face Cardiac Rehabilitation Group Exercise Classes – held at:

- The Centre South Oxhey – Watford and Three Rivers

- Bennett’s End Community Centre – Dacorum

- Harpenden Memorial Hospital – St Albans & Harpenden

- Potters Bar Community Hospital – Hertsmere

  • Virtual Cardiac Rehabilitation Group Exercise Classes and Home Based – Individual Exercise Programme

Prior to attending group exercise classes patients will be required to attend a face-to-face initial assessment with a member of the cardiac rehab multidisciplinary team at one of the clinics listed below:

- Skidmore Way Clinic – Watford & Three Rivers

- Bennett’s End Community Centre and/or Marlowes Health and Wellbeing Centre – Dacorum

- Harpenden Memorial Hospital – St Albans & Harpenden

- Elstree Way Clinic – Hertsmere

We offer a range of treatments to enable you to achieve your goals, this includes:

  • An 8-week Cardiac Rehabilitation Group Exercise Programme (Face to face, Virtual or Home Exercise Programme)
  • A 6-Week Virtual Group Education Programme – delivered live by specialist healthcare professionals
  • Specialist Wellbeing Support – delivered by our Specialist Occupation Therapists
  • Dietary Advice and Support – managed by a Specialist Dietitian
  • Onward referral to long-term management of health programmes, based in the community, supported by local heart charities and support networks.
