Transforming patient care in collaboration with West Hertfordshire Teaching Hospitals NHS Trust

West Hertfordshire Teaching Hospitals NHS Trust and CLCH have been working together on the implementation of the first Covid virtual hospital in the UK. During the early stages of the pandemic, the hospital designed an out-of-hospital care model for patients with COVID-19. Our specialist community nurses and health visiting teams in Hertfordshire played a pivotal role in working as part of a multi-disciplinary team to provide holistic assessments to patients who had been discharged from hospital in the comfort of their own homes.  

This innovative model of care uses remote monitoring equipment and is supported by a centralised clinical and administrative team enabling the multi-disciplinary team to closely monitor patients via an app. So far, WHHT and CLCH have successfully supported over 4,000 patients saving valuable bed space and relieving pressure on services.

It is currently being applied to treat additional conditions that would be suited for treatment in a virtual hospital. The virtual hospital has allowed patients to recover at home and other Trusts around the country are now adopting this model of care.
