Personalised care, tackling inequality and trail-blazing virtual wards: read CLCH’s 2023 annual review magazine

Our 2023 annual review magazine “Community Matters” launched today at our Annual General Meeting (AGM) and is now available to read.

The magazine is packed full of interesting and inspiring stories of how our staff across London and Hertfordshire have used digital technology to provide more personalised care to patients and collaborated with partners to offer a more joined up, holistic experience to service users.

Amid record demand for NHS care, helping people of all ages to stay well and live independently in their own homes and communities has never been more crucial. From improving the nutrition and hydration of care home residents, to empowering children to support their friends with diabetes, and bringing hope to vulnerable families seeking asylum, there’s a wide variety of compelling and informative stories about how CLCH staff go above and beyond every day to deliver great care closer to home.

As the largest community healthcare trust in the country we work with partners across the NHS, local government and the voluntary sector to create healthier communities and tackle health inequalities.
