Nominations open for patient award category
The CLCH Staff Awards are back!
The Awards are a valuable opportunity for CLCH employees to come together, celebrate key achievements and recognise a small number of the exceptional individuals/teams that make up our Trust.
Time to nominate
The patient award is exclusively open to patient and service user nominations. This award recognises an individual/team who has made the biggest difference to patients and service - so we need you.

To nominate via email, send a completed nomination form to clch.communications@nhs.net.
Please note that by submitting a nomination, you are consenting to us contacting you should your nominee win the award. You can nominate as many people as you would like, however you will need to submit a new nomination each time.
Nominations close midnight on Monday 5 August 2019. Please ensure you submit your nomination before the deadline.
