Our medicines management team have won a top prize at the Pharmacy Management National Forum on Medicines Optimisation.
The team won the prize for 'Best Medicines Optimisation in Primary Care', as part of their work on the Proactive Care Homes Project. Congratulations team!
Since 2013 CLCH have been commissioned to provide care to care homes in West London and Hammersmith & Fulham. From the very beginning of the project our medicines management and pharmacy team have shown to improve: safety of medication use; optimise medication through medication review; as well as support care staff with education and training around medication. As well as this, the team also demonstrated significant cost savings.
The team won the award after submitting their work to the Pharmacy Management Forum on Medicines Optimisation to highlight their practice and impact in the care homes. In a nutshell, medicines optimisation is about ensuring that the right patients get the right choice of medicine, at the right time.
The ceremony took place at the Radisson Blu Portman Square Hotel in London on Wednesday 16 November.
