The video follows one patient’s journey to recovery, as she and her OT work together to restore her independence after being discharged from the hospital.

The ESD team is multidisciplinary and includes physios, occupational therapists, speech language therapists, psychologists, community nurses and rehab support workers, who provide critical rehabilitation services for stroke patients.

By working with patients in their home environment, the therapists focus on helping patients regain physical function and confidence, all while surrounded by the familiarity and support of their loved ones.

For this patient, Jacky, home-based rehabilitation played a significant role in her recovery. With her husband by her side, she found comfort in the process. Reflecting on her experience, Jacky shared how the occupational therapist’s approach empowered her throughout her recovery.

“They always asked me, ‘What can we do for you?’ rather than saying, ‘This is what we’re going to do,’” she said. “It was always about what was best for me.” Jacky, patient

The video captures this patient-centred approach, highlighting the tailored care provided by the OT and the impact it had on the patient’s ability to reclaim her independence.

By offering rehabilitation at home, the ESD team not only reduces the pressure on hospitals but also ensures patients can recover in an environment that feels safe and familiar. The video emphasises how this personalised support system helps to build stronger relationships between patients and healthcare professionals, resulting in better outcomes for stroke survivors.

This inspiring story highlights the vital work of occupational therapists across the Trust and underscores the importance of compassionate, patient-led care in the recovery journey.
