Local MP Stephen Hammond visits The Nelson Health Centre
On Friday 2 December, we were delighted to welcome Stephen Hammond, MP for Wimbledon, Raynes Park, Morden, and Motspur Park, to the Nelson Health Centre in Merton to learn more about our Holistic Assessment and Rapid Investigation (HARI) service.
On a tour of the service, Stephen Hammond met Liz Chapman, Clinical Business Unit Manager for Merton Unplanned Care, Jummy Dawodu, Director of Operations, and Susan Brooker, Clinical Operations Manager for Merton Unplanned Care, to talk about the preventive care services that are available to local residents. He also spoke to patients and clinical staff who contribute to the running of the service. Stephen witnessed first-hand how the service keeps people healthy and reduces pressure on the NHS by helping to avoid unplanned trips to hospital.
Jummy Dawodu, Director of Operations for CLCH's South West Division said: “It was a pleasure to give Stephen Hammond, MP a tour of the Holistic Assessment and Rapid Investigation and Falls and Rehabilitation Services we provide for patients and local residents. He spoke with some of our clinical staff and managers and was extremely impressed by the work we have achieved in the service.”
The service is a clinic-based multi-disciplinary community service providing a holistic approach to the complex medical and rehabilitation needs of adults aged 18 years and above registered with a Merton GP. It aims to keep people well, prevent unplanned hospital admissions, promote health and wellbeing, reduce the risk of falls and enable independent living.
