CLCH confirmed as integrator in four boroughs to help drive forward neighbourhood health in North West London
CLCH has been appointed the host integrator in Kensington & Chelsea and Westminster (with GP federations and the voluntary sector taking forward some enabling functions), Hammersmith & Fulham and Harrow in North West London. This is alongside our role with Barnet GP Federation as host integrator for Barnet in North Central London. In Brent, CLCH will continue to be active in the borough partnership.
The integrator model is designed to strengthen how local partners work together to deliver more joined-up, community-based health and care. Integrators will help to bring NHS organisations, councils, primary care and the voluntary and community sector together to build shared understanding of local need and support care that is more proactive, connected and centred around people and neighbourhoods.
The integrator arrangements are:

In the bi-borough (Kensington & Chelsea and Westminster), a partnership between CLCH, Healthcare Central London GP Federation, London Medical Associates, One Westminster, Kensington & Chelsea Social Council (KCSC), Royal Borough of Kensington & Chelsea, Westminster City Council, Imperial College Healthcare NHS Trust, Chelsea & Westminster Hospital NHS Foundation Trust and Central and North West London NHS Foundation Trust (CNWL). CLCH will host the arrangement, with the GP federations and voluntary sector taking forward some of the enabling functions.

In Hammersmith & Fulham, a partnership between CLCH, Chelsea & Westminster Hospital, Hammersmith & Fulham PCNs, Imperial College Healthcare NHS Trust, West London NHS Trust, Hammersmith & Fulham council and Sobus. CLCH will host the arrangement.

In Harrow, a partnership between the Harrow Borough Based Partnership, including CLCH, CNWL, LNWUHT, Harrow Council, GPs and the voluntary and community sector. CLCH will host the arrangement.

In Brent, a partnership between CNWL, London North West University Healthcare NHS Trust (LNWHUT), Brent council, CLCH, the voluntary sector and Brent primary care networks (PCNs). CNWL will host the arrangement.
This important step also reflects the Government’s ‘three shifts’ outlined in the 10 Year Health Plan: to move care from hospitals to communities, shifting focus from treating sickness to preventing illness, and transitioning from analogue to digital systems.
It is also in line with CLCH’s five-year strategy, launched in July 2025, which aims to strengthen community care as the cornerstone of health and wellbeing in the populations we serve.
CLCH is committed to moving beyond traditional healthcare delivery and using every lever at our disposal to improve population health outcomes. This involves enhancing the physical and mental health of entire communities, reducing health inequalities, and addressing the wider factors that affect health. Critical to achieving this is working closely with communities, partner organisations, healthcare professionals and residents to provide more effective, preventive, and inclusive care.
