Brent Diabetes team shortlisted for innovation at HSJ Digital Awards 2025
The Brent Diabetes Team has been recognised for their innovative approach to diabetes care, having been shortlisted for the HSJ Digital Awards 2025 in the category of Generating Impact in Population Health through Digital.
The HSJ Digital Awards celebrate excellence making use of digital solutions to improve patient care, drive efficiency, and transform healthcare services. The Generating Impact in Population Health through Digital category specifically highlights projects that use digital tools to enhance healthcare outcomes for entire communities, ensuring services are more accessible, effective, and tailored to local needs.
The Brent Diabetes team earned their place on the shortlist for their innovative work using digital continuous blood glucose monitoring to support people with Type 2 Diabetes, particularly those on injectable therapy. By partnering with GP practices and local community services, the team has focused on improving key health measures – blood sugar levels (HbA1c), cholesterol, and blood pressure – with a particular emphasis on helping patients with very high blood sugar levels bring them down safely.
Through digital technology and education, the initiative has equipped patients to take better control of their diabetes, aiming for a 20% reduction in HbA1c within six months. This approach not only improves individual health outcomes but also reduces the long-term strain on NHS services by preventing diabetes-related complications.
