Brent Diabetes Service shortlisted for a Quality in Care Diabetes Award
Congratulations to our Brent Diabetes Service who have been shortlisted for Quality in Care (QiC) Diabetes Award in the Improvements in Diabetes Care Using Data category.
QiC Diabetes Award recognises, rewards, and shares innovative practice demonstrating quality in diabetes management, education, and services for people with diabetes and/or their families.
A Quality in Care award means an initiative has been recognised by the NHS, patients and industry as improving the quality of life for people living with diabetes.
The Brent Diabetes Service work collaboratively with local communities in primary care to meet the needs of the local population using data from Northwest London dashboard and National Audit data.
The aim of the project is to improve education uptake for newly diagnosed and ongoing people with Type 2 diabetes and injectable therapy, optimise medication, support care home staff, clinical pharmacists, and work towards minimising health inequalities.
Anganie Seecharan, CLCH Brent Diabetes Clinical Service Lead said: I am so proud of the team – it really is a huge collaboration effort between all partners involved including GPs, Integrated Neighbourhood teams, Pharmacists, North West London ICB and more. Our teams work very hard to continue improving health outcomes for people with diabetes, and being shortlisted is a fantastic achievement.”
