Barnet care homes to benefit from enhanced support services

From July 2016, Central London Community HealthCare Trust  are working with Barnet CCG and Royal Free London Foundation Trust to pilot a team working in 10 care homes to prevent emergency admissions.

This is high profile project called CHESS - Care Homes Enchanced Support Services - is specifically aimed to improve outcomes for patients and to improve use of health services by care homes in Barnet. It is a small start to a large issue - as there are over 100 care homes in the borough.

The new multidisciplinary team comprises of Nursing, Physiotherapy, Occupational Therapy, Pharmacy and a Consultant Geriatrician and will work alongside the GP and care home staff to provide a mixture of hands-on care and training. By focussing on more complex patients identified through risk stratification we will improve quality of care and introduce preventative pathways to support early identification of deterioration.  The group will also have holistic medication reviews to reduce polypharmacy and waste.
