I am submitting this FOI request to gather insights into current processes around technologies regarding discharge and handovers of care. Please find a set of questions below:
- Do you currently have an integrated discharge hub/lounge?
Yes.
- Which organisations are involved in your integrate discharge teams?
Transfer of Care Hub (Central London Community Healthcare NHS Trust) and Complex discharge team (London North West University Healthcare NHS Trust).
- Do you currently manage discharges with any tech system in place?
Yes, as an integrated discharge team, we use OPTICA.
- Are you currently using OPTICA?
Yes.
- If yes to question 3, when does your contract renew?
Not applicable as supplied by third party.
- If yes to question 3, what is your annual expenditure of your contract?
Same as response to question 5.
