1. Does the Trust use a third-party provider to create/write, manage or track patient information leaflets? - None applicable to the trust as we do not have a leaflet production and tracking system, patient information leaflets are managed locally by the services.
2. Does the Trust have a software solution to manage patient e-consent? - E-consent system for children’s vaccination services
a. If yes, is this a standalone solution, or part of a wider electronic patient records system? - it is a stand alone solution.
3. For the Trust’s patient e-consent system(s), could you please provide the following information (if the Trust uses multiple third-party solutions, please provide details for all of these):

Contract(s) renewal date(s) - This contract ends 31st August 2024. Contract will not be renewed as CLCH will no longer be delivering the service from 1st September 2024.
4. When did the Trust first start using this third-party? - The contract with Cinnamon started on the 31st March 2020.
5. Please could you indicate which solutions are used by each department in the Trust: - Central London Community HealthCare NHS Trust (CLCH) specialises in providing care in the community. CLCH does not provide the below listed services.
