If yes please answer the following:

1. The total of physical sites within the Trust?
2. Which trust/NHS organisation do they reside at?
3. How many patients did each of the identified clinics support in FY 2022-2023?
4. What treatment programmes are provided in each of the identified Child Obesity clinics?
5. How are these treatment programmes assigned, e.g. home visits, clinics?
6. How long do treatment programmes continue for?
7. Within these Child Obesity clinic’s, what digital solutions are in place to support the care pathway?
8. Name of solution
9. Name of solution supplier
10. Implementation date
11. Contract renewal date
12. Do these digital tools provide a two-way communication between the patient(parent) and clinician?
13. If yes, what additional features do these solutions provide to the patient and clinician?
14. Does the trust have plans to advertise a tender to replace these solutions?
15. If so, confirm the time frame.
