Thousands of CLCH patients to benefit from healthcare on their doorstep

Thousands of people are to benefit from healthcare on their doorstep, as Westminster, Kensington and Chelsea and South and West Hertfordshire are chosen as pioneers for national neighbourhood health rollout.

This week, the government announced 43 areas in the country that would be part of the first wave of the neighbourhood health service roll out.

Two of these areas – Kensington, Chelsea and Westminster and South and West Hertfordshire – are areas in which CLCH is a key partner for service delivery.

Neighbourhood health is a way of delivering care that brings together a range of healthcare providers, social care, and voluntary services to support people where they live, giving people easier access to the right care and support.

This kind of care is designed to better support people with long term conditions, such as diabetes, frailty, arthritis, or high blood pressure, in areas with the highest deprivation.

These areas have been chosen as pioneers of the neighbourhood health service due to strong partnership working among existing local services.

Being part of the national programme will enable local services to accelerate key initiatives and work towards a more relational and holistic approach to care with communities. This will help patients avoid unnecessary trips to hospital, prevent complications and avoid the frustration of being passed around the system.

Pioneer areas will benefit from advice, coaching, and learning resources from national experts.

As part of CLCH’s strategy 2025-30, we are committed to enabling a flourishing neighbourhood health service that give children the best start in life and adults greater independence. This announcement is another step towards making that vision a reality for our patients and communities. We will continue to work with partners in all the areas we deliver services in to provide seamless community care and improve the health and wellbeing outcomes for our local communities.

Health and Social Care Secretary Wes Streeting said:

“Neighbourhood health services fundamentally reimagine how the NHS works – bringing care closer to home while helping to tackle this nation’s shameful health inequalities.

“Through our Plan for Change, we will stop people from being bounced around a broken system as we get the health service back on its feet.”
