Integrated partners in Hertfordshire finalists for the 2022 HSJ Awards

South and West Hertfordshire Health and Care Partnership (HCP) - a collective of health, social care and voluntary sector organisations, is a finalist in the 'Place-based Partnership' category at this year's Health Service Journal (HSJ) Awards.

The service has been recognised for its outstanding contribution to healthcare and demonstrate how partners are working together to share best practice and improve patient experience.

The shortlisting for the HSJ Award follows CLCH's collaboration with Hertfordshire Teaching Hospitals NHS Trust in the development of the UK's first Covid virtual hospital. The establishment of the virtual hospital was designed to provide direct integrated care for patients with COVID-19 when discharged from hospital and to continue their recovery at home safely.  

Working as part of a multidisciplinary team, our dedicated and professional community nurses and health visiting teams in Hertfordshire provided comprehensive assessments such as monitoring patients’ blood pressure and heart rate as well as visiting patients at their homes regularly.

Commenting on the shortlisting, Interim Director of Operations for the Hertfordshire division, Leanne Fishwick said, “I am absolutely delighted that the work put into designing, establishing, and implementing the virtual hospital has been recognised with this shortlisting. This is a fantastic development for patients, as it provides seamless care through integrated working practices.”
