Working together with Care UK in Harrow to achieve better outcomes for patients in the community

Buchanan court.jpgOur Harrow team are working together in partnership with Care UK to deliver inpatient rehabilitation for NHS patients within two Care UK homes in Harrow – Woodland Hall and Buchanan Court. Care UK provide all the nursing care whilst CLCH provides the In Reach rehabilitation therapy.  The multi-disciplinary teams work together to ensure that patients are assessed and triaged to achieve the best possible outcomes.

Our In Reach therapy team is made up of physiotherapists, occupational therapists and rehabilitation assistants all supported by a Care of the Elderly consultant.  The therapy is focused on helping patients achieve their goals through 1:1 sessions, group exercise classes and group activities.

Christine Chaffin, our In Reach therapy lead advocates partnership working in the community.  She believes, “The joint working structure has given both sites a unique atmosphere. Providing therapy in a community care home setting has helped to motivate patients and improve their independence helping them to achieve their therapy goals.”
