Changes to Inner London referral routes

From 1 November 2016, queries relating to CLCH community nursing, bedded rehabilitation, neuro-rehabilitation and falls will be processed through a single telephone number - 0300 033 0333.

The new 0300 033 0333 telephone number is to be used for queries only for these services.

The CLCH Single Point of Access (SPA) remains the first point of contact for referring or booking appointments for the following services:
 

• Diabetes - Kensington & Chelsea, Hammersmith & Fulham and Westminster
• Diabetes Podiatry Service - Hounslow
• Tissue Viability - Kensington & Chelsea and Westminster
• Podiatry - Kensington & Chelsea, Hammersmith & Fulham and Westminster (self-referrals are accepted)
• Bladder and Bowel -Kensington & Chelsea  
• Nutrition and Dietetics - Kensington & Chelsea, Hammersmith & Fulham and Westminster 
• Specialist and Weight Management -  Kensington & Chelsea

What happens when a clinician or patient calls the new 0300 number?

Upon calling this number, callers will be asked to select from a choice of the Community Independence Service (CIS), or CLCH's community services within the tri-borough area. Callers selecting an incorrect option for the service they require will subsequently be transferred to the appropriate destination.
