Brent Health Matters wins HSJ Patient Safety Award as Tissue Viability team highly commended

We are proud to announce Brent Health Matters (BHM) team has won the HSJ Patient Safety Awards 2025: Learning Disabilities Initiative of the Year.

The award was given for their project - The role of pharmacists in supporting the Triple Aim Framework for individuals with learning disabilities.

The team’s work focuses on reducing health inequalities in Brent by building stronger links between healthcare professionals, social care providers and the local community. They reach out to groups who often face barriers to healthcare, including people with learning disabilities.

Pharmacists in the team play a key role by carrying out home-based structured medication reviews (SMRs). These reviews make sure that medicines are prescribed safely, are right for each person’s needs, and are clearly explained. This helps people understand and take their medicines correctly, which leads to better health and quality of life.

Jackie Allain, Director of the Outer North West Division, said:

“I am immensely proud of our teams being shortlisted this year, including Brent Health Matters and the Looked After Children team. A huge congratulations to BHM for winning this award. It is so well deserved as the team make such a difference to the patients of Brent every day.”

Alongside this success, the Inner North West Tissue Viability team also received highly commended recognition in the Patient Safety Education and Training Award for their project - Addressing skin tone bias in wound care in community practice.
