Brent Health Matters Highly Commended in North West London Health Equity Awards

Brent Health Matters (BHM) team has been recognised with a Highly Commended honour at the North West London Health Equity Awards which is run by North West London Integrated Care System, celebrating their exceptional efforts in reducing health inequalities and improving access to healthcare in the region.

The accolade highlights the tireless work of the BHM team, which includes GPs, nurses, healthcare coordinators, dietitians, and pharmacists. This multidisciplinary team addresses barriers to healthcare by engaging with patients who often face challenges in accessing health and social care. By providing holistic health checks and personalised support, BHM empowers individuals to better manage long-term conditions such as hypertension and diabetes.

In a statement of gratitude, Gopal Patel, Specialist Health Inequalities Pharmacist said: “Our mission is to tackle health inequalities in Brent’s most deprived areas. Through referrals from GPs, social care, and even homeless shelters, we aim to re-engage those who might otherwise be left behind. Outreach events like our recent initiative on World Hypertension Day targeting the African-Caribbean community help us raise awareness and encourage open conversations about health. These efforts have led to identifying serious health concerns such as hypertension, hyperglycemia, and atrial fibrillation in the community.”

The North West London Integrated Care System commended BHM’s innovative and compassionate approach, noting the incredible impact such grassroots initiatives have in promoting equitable access to healthcare.

This recognition underscores the significant strides being made by the BHM team in creating a healthier, more inclusive community.
