Barnet community to benefit from a new partnership heart failure service

CLCH's community service team in partnership with the Royal Free London NHS Trust will be providing a new community heart failure service to its patients in the area. This service goes live on 6 June 2016.
The heart failure specialist nurses will ensure that all patients receive evidence based treatments. Patients will be provided with service contact details, written and verbal information around the management of heart failure, given education regarding self-management and recognising early signs or symptoms of heart failure and when to contact the service for advice or a home visit. Patients will be seen in their homes or community clinics.
The service working hours will be Monday to Friday 9am to 5pm (excluding bank holidays). Patients can be referred to the service by GP's, secondary care and other health care professionals on the designated service referral form. Patients must have a diagnosis of heart failure confirmed by an echocardiogram.
The service will consist of heart failure specialist nurses, physiotherapist, admin staff, clinical psychologist with clinical support and access to a consultant cardiologist.
The heart failure nurse specialists will work closely with Barnet GP's and cardiologists from the Royal Free London NHS Trust. This service has been commissioned by Barnet CCG.
