Request

1) We have the following formularies listed as used by the organisation:

North West London Integrated Formulary

Please confirm if this is correct and if not, please provide any relevant online links or copies of formularies.

Yes, we adhere to the formulary. North West London Integrated Care Board (NWL ICB) is the owner of the formulary. Copies and online link will need to be requested from them.

 

2) We understand the following committees maintain the formularies in question 1:

North West London Integrated Medicines Optimisation Committee

Please confirm if this is correct. If not correct, please list the relevant committees with the corresponding formulary. - Correct.

 

3) Does the organisation have representation on the committees listed in question 2? - Yes

 

4) Are any of the above formularies listed in question 1 dominant when making prescribing decisions or does this depend on therapy area? Please say which is the dominant formulary of the organisation. - There is only one formulary listed.

 

5) If you use a PDF formulary, have you considered moving it to an online platform like Netformulary, and if yes, when are you planning to do so? - Please refer to North West London Integrated Care Board NWL ICB  for response to this.
