Please see responses to the following FOI request. Please note that I have not complied with the request to provide the number of deaths that were reviewed in 2023/24 where families and carers were involved and engaged in the review as this would involve me having to review 18 mortality case reviews which I do not have the capacity to do within the deadline period (before the 11/3/25) due to other commitments. However, all other queries have been responded to. Apologies for the formatting - but I think the information is still legible:

1a. Does your trust currently comply with the requirements of the Leaning from Deaths Guidance?
1b. If not do you have plans to comply with it and if so by when?

2a. How many deaths were reported within your trust during the reporting period (2023/24)?
2b. How many of these deaths were reviewed, and how many were judged more likely than not to have been avoidable?
2c. Have you published quarterly reports on learning from deaths on your website, as required by the guidance? Please provide links to the most recent reports.
2d. Information on learning from deaths included in your latest Quality Account?
Family Involvement
3a. Are families and carers involved in the review process where appropriate?
3b. Please provide the number of deaths that were reviewed in this period (2023/24) where families and carers were involved and engaged in the review.
Actions and Learning
4a. Please provide a summary of all actions taken as a result of the learning from deaths process in the last reporting period.
Response
I would like to confirm Central London Community Healthcare hold the data that is being requested. However, the data for question 3b is not separately recorded and reports will have to be reviewed manually to extract the information. As we do not record the information in an easily, retrievable format, it I estimated that it will take over 18 hours to extract, analyse, clean and calculate the data to provide a response for this question.
We will therefore apply FOI Exemption Section 12 of the FOI Act 2000 (Cost Limit).
Due to the excess cost, it removes the Trust’s obligations under the FOIA to your request. However, under our duty under section 16 to provide assistance and advice, it is possible to respond to your request, omitting question 3b. If you do require a response, please do get in touch using the reference above.
