I am seeking information on the provisions in place at Oxgate Gardens Surgery, 81 Oxgate Gardens, NW2 6EA.

1. Please provide the names of all Doctors registered to practice at Oxgate Gardens Surgey, their shift pattern and whether that includes being in the surgery or away from the surgery (working from home for example).
2. Please state if any of these Doctors are registered at another practice or work privately.
3. Please provide the medical qualification for Ernest Mumbwatasai, the advanced nurse practitioner, as it not showing on the NHS page for this survey.
4. Please state how many patients are registered at Oxgate Gardens Surgery.
5. Please state how much, per registered patient, is paid to Oxgate Gardens Surgery every year.
6. Please state the threshold for the Patchs system to be switched off.
7. Please state the average length of time the Patchs system is open per day since 1 January 2024.
8. Please state how many face to face appointments with a registered GP ( not nurse or advance nurse practitioners) are available to be booked at Oxgate Gardens , broken down per day through Patchs.
