Please see below requested information from Employee Health Service (also known as Occupational Health).
- Information on the following variables in any format you may have on file preferably a spreadsheet with a line for each identified case (please feel free to use the attached spreadsheet if you find it easy to navigate and does not create extra work for you):
1. What are your organisational top three reasons for staff health/wellbeing occupational health referrals in ascending order (this question is not on the spreadsheet)?
2. What was the number of sickness absence days in 2020-21, 2021-22, 2022-23, 2023-24, 2024-25 (line by line cases in each of these years) due to:
§ Work-related stress, anxiety, or other mental health conditions, and alcohol or substance misuse conditions
Employee Health Service do not have access to this data. We cannot provide the People Analystics team with the names and reasons for referral of employees to Employee Health to enable them to provide this data as this would be a breach of medical confidentiality.

3. For each identified case in b) above was a phased return to work recommendation made?
This information is not easily accessible and this would required serveral days to manually extract

10. Turnover rates (retirement on health grounds Yes/No)
- A copy of your current staff wellbeing policy, including any initiatives or frameworks in place to support the mental health and wellbeing of community nursing staff. This includes self-referral services which staff are encouraged to use in confidence without the organisation’s knowledge.
