1.1 Does your Trust / Health Board have an orthotics service and/or a prosthetics service? (Please tick yes even if the service is contracted to a private provider)

*Please note government related body refers to any public funded body which has provided the Trust/ Health Board with funding for the prosthetics and/or orthotics service.  The total declared should match the total funding received to deliver the service before any costing for Trust/Health Board overheads were taken.
