Please can I request a copy of the impact assessment that was carried out in relation to the decision to decommission the Community Parkinsons Disease Nurse Specialist service, affecting Westminster patients. Can I also request a copy of any letters sent out to patients informing them of the closure of the service (without any patient details on - just the text of the letter itself) and minutes of any meetings where the closure was discussed.
