The health improvement team provides a range of services to help improve health and wellbeing and reduce health inequalities in the local population.

The team are an experienced team of health promotion practitioners including community nurses, physical activity specialists, and a support worker. The team work in partnership with statutory and third sector organisations and local residents.
 

Who we see

Our health improvement services sees adults who live in Westminster or are registered with a GP in Westminster.

The service does not see children or adolescents.

Who can refer?

The service accepts referrals from GPs, clinicians, partners and also self-referrals.

Services Description Referral

Healthy lifestyle checks

30 minutes health checks in the community (e.g. in libraries) for those unwilling to have or are ineligible for an NHS Health Check.

Each consultation includes blood pressure, cholesterol and BMI measurements, goal setting and signposting to other services. Follow up service also available.

Making Every Contact Count (MECC) Training

Level one (half day) and Level two (two days) training in brief health promotion interventions (based on motivational interviewing) for frontline staff and volunteers.

The training develops communication skills in delivering brief health promotion advice on physical activity, healthy eating, smoking and alcohol. The course is accredited by the Royal Society for Public Health.

Support with accessing health, housing and community services for vulnerable adults

(e.g. adults with chronic mental health issues or at risk of homelessness).

We deliver physical activity sessions for adults with learning disabilities and/or longstanding mental health issues (e.g. gym sessions at the Paddington Recreation Ground).
