What is a Community Matron?

A Community Matron is an experienced senior nurse who supports people with complex health needs. They work closely with patients, families, carers, and other healthcare professionals to help people stay as healthy and independent as possible in their own homes.

How can a Community Matron help?

Our Community Matrons provide personalised support to help manage your health and wellbeing. They can:

Assess your needs

We take time to understand your physical health, emotional wellbeing, daily activities, and any support you may need.

Create a personalised care plan

We work with you to develop a care plan that reflects your needs, goals, and preferences. We regularly review your plan and make changes when needed.

Coordinate your care

We act as a main point of contact and work closely with your GP, hospital teams, therapists, social care services, and other healthcare professionals to make sure your care is joined up.

Support with medicines

We can help you understand your medications, answer questions about your prescriptions, and support you to manage your medicines safely.

Provide advice and education

We offer information and guidance about managing long-term health conditions, healthy lifestyle choices, and ways to look after yourself at home.

Monitor your health

We keep in regular contact through home visits, telephone calls, or other methods of communication to help monitor your condition and respond to changes early.

Support you and your family

We provide advice, reassurance, and support for carers and family members who are helping to care for you.

Consider your whole wellbeing

We look at all aspects of your life, including your physical health, mental wellbeing, social circumstances, and support networks.

Help with future care planning

We can support discussions about your future care wishes and help you make plans that reflect what matters most to you.

End-of-life care support

For people living with severe frailty or advanced illness, we help coordinate care that focuses on comfort, dignity, and quality of life.

  • Work closely with healthcare and social care teams

Referrals

Please note, the referral process remains the same:

  • Patients meet criteria for referral (see attached)
  • Email sent to SPA clcht.spa@nhs.net using the needs-based assessment form.
