Our vision is to provide great care closer to home.

Our values guide how we work and support each other every day:

We have four core values and linked behaviours we aim to meet at all times when working with patients, partners and colleagues. Our values guide how we work and support each other every day:

Accountability:

  • We take ownership – individually and collectively – for our behaviour and for the impact we deliver for our patients, neighbourhoods, and the wider system.
  • We take responsibility, are transparent with colleagues, and learn from mistakes without placing blame.

Inclusion:

  • We respect differences and ensure all voices are heard in decision-making and care planning.
  • We lead with integrity, curiosity, and openness and create a sense of belonging for each other and our patients.
  • We ensure services are accessible to all and adapt care as required to meet the needs of our communities.

Compassion:

  • We show kindness, empathy and understanding to patients and colleagues and build trusted relationships.
  • We listen without judgement, understand people’s lives and circumstances, and treat people holistically to reduce health inequalities and support wellbeing for all.

Empowerment:

  • We support patients and families to take an active role in their care, decision making and managing their health.
  • We work collaboratively, recognise colleagues’ achievements, and promote a reflective learning culture.

Our strategy

In July 2025 we launched our new five-year strategy to improve the health and wellbeing outcomes of our patients and communities. The strategy ‘Healthy Neighbourhoods, Thriving Communities’ is a radical step forward, positioning community care as the cornerstone of health and wellbeing in the populations we serve.

You can learn more about our strategy by watching our animation video on YouTube.

The full strategy document can be downloaded from on our website.

The CLCH Academy provides education and training opportunities which enable all community and primary care professionals to deliver effective and compassionate care.

It is a place where staff can learn together, gaining skills, knowledge, academic accreditation and professional support to help them grow and develop their careers.

Watch this video on YouTube to find out more about the Academy at Soho.

We aim to adapt to our learners' needs, developing learning interventions and educational programmes led by our expert faculty of clinical educators and our partnership with London South Bank University.
