Empowering communities through innovative diabetes care

The new Trust values are Accountability, Inclusion, Compassion, and Empowerment. These have been designed by our staff to guide our behaviours, shape care delivery, and help us build trusted relationships with patients, communities, and each other.  

Our values were published in our new Trust Strategy 2025-30 which launched in July 2025 and support our mission to enable thriving communities that give our children the best start in life and adults greater independence.  

The value of empowerment means supporting patients and families to take an active role in their care and managing their health, while promoting recognition and a reflective learning culture with colleagues.  

The Brent and Harrow integrated diabetes teams at CLCH deliver high-quality, specialist diabetes care in community settings, helping people with Type 2 diabetes gain the knowledge, skills and confidence to manage their condition effectively. Working closely with GP practices and local partners, the teams offer a wide range of clinics including multidisciplinary community diabetes clinics, consultant clinics, group insulin therapy and link diabetes nurse support. 

A key part of this diabetes care is the DESMOND (Diabetes Education and Self-Management for Ongoing and Newly Diagnosed) programme. Delivered face-to-face, online or through the MyDESMOND digital platform, DESMOND empowers people to understand their diabetes, make informed lifestyle choices and take control of their day-to-day care. 

This strong focus on empowerment and innovation has recently been recognised nationally. The Brent Diabetes Service was shortlisted for the Quality in Care (QiC) Diabetes Award for using data to improve education uptake and for the HSJ Digital Awards 2025 with their innovative use of continuous glucose monitoring to support patients on injectable therapy.  

Anganie Seecharan, Brent Diabetes Clinical Service Lead, said: 

“Empowerment means giving people the confidence and the tools to make decisions about and manage their own health. In my week, that looks like supporting my team, working with partners, and most importantly helping patients feel informed, capable and in control of their diabetes.”
