    The value of empowerment means supporting patients and families to take an active role in their care and managing their health, while promoting recognition and a reflective learning culture with colleagues.   The Brent and Harrow integrated diabetes teams at CLCH deliver high-quality, specialist diabetes care in community settings, helping people with Type 2 diabetes gain the knowledge, skills and confidence to manage their condition effectively.
